About

Monroe Canyon Assisted Living is a 27-bed assisted living community in Monroe, Utah that opened in 2009. It aims to support an independent but supervised lifestyle in a home-like setting, with private rooms and private showers, 24-hour supervision, personal care, medication management and meals served in a family-style dining area; an RN coordinates a wellness plan for each resident. Common areas include a large commons and dining area, a day room with a fireplace and piano, a hair salon, an activity room, and gardens and walkways. Home health, physical therapy, podiatry and hospice services are available on site as needed, and an Intermountain Health clinic leases part of the building.

State inspection record

Clean record

No citations in the state’s three-year record, across 2 inspections.

44% of licensed Utah communities have no citations in the state’s three-year record.

A citation is the more serious of the two notices Utah issues for a rule out of compliance. The lesser one, a warning, is not published by the state, so nothing on this record is at that level.

Utah assigns each finding a risk level — Low, Moderate, High or Extreme — based on the likelihood and severity of harm that could result if a rule is out of compliance.
