For hospitals, clinics and discharge teams
The discharge list, made informed.
Hand the family one link. They see every home health agency, hospice or community that serves their area, with the facts behind each, and your team’s picks named above the list. Free. Nothing about the patient leaves your building.

What the family gets
The whole list
Every Medicare-certified agency and every licensed community that serves their area. Not a sample, not the ones who pay.
Your picks, by name
“Recommended by your care team” sits above the list with your organization’s name, and any relationship disclosed. Never instead of the list.
The facts and the tools
Certification, survey history, insurance by plan, ratings, and the guides and quizzes you choose to send along.
How it works
Mark your team’s picks once
Per kind of care. Say if there’s a relationship. Save a packet for the sends you make every week.
Make a link for the family
Kind of care, county, their plan. A QR code for the discharge folder or an email they asked for. No name, no diagnosis, nothing about the patient.
The family chooses
On their own phone, with the full picture. If they choose to keep you posted, you’ll know when they decide. Nothing else, and nothing unless they say so.
The rules we keep
- Free for your team, free for the family. Nothing is counted per link, per family or per placement.
- No money in any direction. No provider can pay to be recommended, and nobody pays you. We never share provider revenue with a referrer.
- The list is the list. A provider’s plan with us changes nothing you see and nothing the family sees from your link.
- The patient stays with you. The link carries a search, not a person. The family decides what, if anything, you learn back.
Set up your team
For hospitals, clinics, physician groups and discharging facilities. Not for paid placement agents. A person confirms each organization, usually the same day.
