State inspection record
Findings corrected
1 finding in the state’s three-year record, none of them rated High or Extreme.
Most licensed Utah communities have 1 or fewer citations in the state’s three-year record.
A citation is the more serious of the two notices Utah issues for a rule out of compliance. The lesser one, a warning, is not published by the state, so nothing on this record is at that level.
Utah assigns each finding a risk level — Low, Moderate, High or Extreme — based on the likelihood and severity of harm that could result if a rule is out of compliance.

Low Resident's legal rights(R432-270-9(2)(a)-(b))
The Licensee was out of compliance with R432-270-9(2)(b) by not including all required statements in the admission paperwork. During the inspection, the licensor reviewed the paperwork given to each resident upon admission. The paperwork did not include a statement that the resident could file a complaint with the state long-term care ombudsman or any other advocacy group concerning resident abuse, neglect, or misappropriation of resident property in the facility. This is a repeat non-compliance as noted on 6/25/2025.
How was this corrected? On September 15, 2025, the licensor verified that the administrator ensured residents were made aware of all their legal rights, as stated in rule.
